TMG uses this form to obtain written consent from field employees and subcontractors before sending work assignment text messages. Signing is optional. It is not part of hiring paperwork or any other agreement, and declining does not affect anyone's job, pay, hours, or work assignments. Signed originals are kept in the personnel file.
